Intake forms
Collect the information you need from patients during booking — assignable per service, with consent capture and an audit trail whenever answers are viewed.
Create a form
- Go to MediBook → Intake Forms and click Add form.
- Give it a name, set Applies to (all services or a specific one), and set the status to Active when ready.
- Add fields, drag-ordered with the up/down controls.
Field types

Dropdown and multiple-choice fields take one option per line. Any field can be marked required, with optional help text.
Where answers appear
Active forms are shown during booking for the services they’re assigned to. Submissions are attached to the appointment and viewable on the Appointment details and Patient screens by staff with the intake permission. Opening a submission is recorded in the audit log, since it may contain medical information.
Tell patients not to include sensitive medical details in the free-text “notes” field — use structured intake questions instead, which are handled with the appropriate privacy safeguards.
